CMO Updates

New Medical Directors

  • Rolf Kreutz, MD has been named medical director for invasive cardiology. Dr. Kreutz joined the IU Health team in 2006 as an interventional cardiologist. He currently serves in a number of leadership positions, such as chief of cardiology and medical director of the cardiac catheterization laboratories at Eskenazi Health, as well as associate program director for the Interventional Cardiology Fellowship at Indiana University. He is also an associate professor of clinical medicine at the IU School of Medicine.
  • Kyle Frick, MD has been named medical director of cardiology for abdominal transplant. Dr. Frick joined the IU Health team in 2018 as an interventional cardiologist. His research interests include clinical informatics, quality improvement, and the effects of cirrhosis on the cardiovascular system. He is also an assistant professor of clinical medicine at the IU School of Medicine.

 

Completion of medical records moving to a one-week suspension cycle
The Health Information Management department will soon begin operating under a one-week suspension cycle rather than the current bi-weekly cycle. This will ensure more timely documentation to meet CMS requirements and Joint Commission standards, support prompt coding/billing of accounts and better align deficiency management, giving providers a more unified experience across the state.

What to Expect:

  • Beginning Jan. 31, providers will receive notification of incomplete medical records that have aged 15 days post discharge.
  • The following Wednesday, Feb. 7, providers who have not yet completed their incomplete medical records will be suspended at noon. Immediately following, Warning Letters will be distributed for any newly aged deficiencies.
  • This cycle will continue weekly.
  • Providers should routinely log into Cerner Message Center and complete all outstanding deficiencies in their “Sign”, “Document” and “Saved Notes” folders.

Don’t Forget to:

  • Change your Cerner Message Center default display from 30 Days to Load All.
  • Refuse incorrectly assigned documents to “Administrator, Refusal HIM Document” for HIM to review.
  • Notify the HIM Chart Completion team prior to a leave of absence to avoid unnecessary suspensions.

Impending shortage of N95 respirators
There are two product disruptions from 3M occurring with N95 respirators: O#77297, 3M 1860, and O#77298, 3M 1860S. There are N95 respirators and PAPR products available as a practice alternative. The SBAR attached has more information.

The N95 respirator must be fit tested to be protective of staff and patients. Here are upcoming fit testing clinics. Walk-ins are welcome.

16th Street set to re-open in January

Demolition work has resumed on the People Mover structure and 16th Street will re-open toward the end of this month.

 

Kicking off 2024 with kudos:

  • Congratulations to Roopa Rao, MD for being selected to the Sandra J. Lewis Mid-Career Women’s Leadership Institute. The program serves to empower mid-career cardiovascular women physicians and support them to flourish during this distinct and exciting phase of the career journey.
  • Cutting edge medicine: Last July, the FDA approved a new Alzheimer’s Therapy named Lecanemab (Leqembi). Lecanemab is one of the first drugs that targets the underlying disease process of Alzheimer’s disease that has shown clinical benefit. Through our new Neuroscience Brain Health Program, IU Health Behavioral Neurologist were the first in Indiana to prescribe and launch Lecanemab as a treatment for qualified patients. Phase 1 of the Program began in September, with an initial 30 patients enrolled for an anticipated 2-year treatment regimen. The team anticipates enrolling 100 additional patients in 2024. Here’s more on how our first patient reacted to the news that he qualified for the drug.
  • In the news: Caring for our community - Julie Clary, MD, discussed risks and symptoms of heart attacks on Fox59 News ahead of the often stressful holiday season, and the importance of seeking care. Marc Rovner, MD, discussed on WFYI how the high rates of COPD in Indiana are often caused by smoking, also reiterating the importance of quitting smoking and treating COPD patients with compassion.

ER Advisor is launching in our ED very soon!

We want to give you all a quick overview of what this platform is all about and how it will help improve patient care.

? The ERAdvisor experience starts when a cell phone is entered into the electronic health record.

? The patient is sent an SMS message to their smartphone.

? Best practice is to collect cell phone numbers on every patient, every time, even the patients that arrive by ambulance. There are smart filters in place to exclude patients who shouldn’t be invited to the platform. Patients who are an ESI of 1 are generally NOT invited to the app.

? Patients should be told that they will receive an SMS message from your hospital and that it is not spam. Please instruct patients to click on the URL or link to enter into the platform. Please remind patients that it is NOT just text message updates. It is a secure website that will keep them up to date about everything that is happening during their ED visit and follow up after they leave the hospital.

Clinical Council Updates

Neurology Clinical Council

  • Stroke One Workflow: Dr. Jason Mackey updated the council on upcoming changes to Stroke One workflows for both Comprehensive and Primary Stroke Centers. Changes are expected to go live in early 2024, pending feedback and direction from each site/region. 
  • 2024 The Joint Commission Disease Specific Standards: Stroke coordinators provided an overview of updated standards from the Joint Commission. Click here for more information on TJC Stroke Certification Standards or contact the Stroke Coordinator for your location.
  • 2023 Neurology Council Year in Review:
    • Developed and implemented Goals of Care documentation for Ischemic Stroke patients age >65 with a NIHSS >10 (=goals_of_care_neurology).
    • Implementation of Lecanemab for the treatment of Alzheimer's. Including use/ordering restrictions, power plan creation, education for post-administration risks, and Cerner alert for providers to avoid use of contraindicated medications.
    • Developed UMG quality metric for epilepsy and reproductive health education. Pending IS implementation.
    • Implemented new Cerner order for Skin Biopsies to improve provider workflow.
    • In collaboration with Stroke Coordinators, implemented several stroke care enhancements within Cerner, including addition of pharmacy consult to stroke order sets to ensure correct med routes, identified standard language for stroke patient education, and build of Stroke quick view tab in Cerner (for nursing).
    • UPDATE: PO med route issue for stroke patients. The pharmacy team is on the end phase of the build for a Cerner Alert for patients that:
      • Have an Ischemic or Hemorrhagic stroke power plan ordered
      • Have a NPO order or an order for tube feeds
    • If a provider orders a PO med with any of the 3 conditions listed above, an alert will fire to notify the provider to either change the route of the medication or to update the diet order. A tentative go-live will be sometime in mid-to end Q1, 2024.
  • Tenecteplase Initiative: In collaboration with System Pharmacy, IU Health has seen a savings (cost avoidance) of nearly $1.3 million since switching from Alteplase to Tenecteplase for management of acute ischemic stroke in September 2022.

 

Pediatric Emergency Medicine

The Pediatric Emergency Medicine Clinical Effectiveness Team has been indefinitely paused due to team leader transitions and capacity. We want to thank Dr. Cory Showalter and Pharmacist Lauren Buenger for their time and expertise. Additionally, we extend our gratitude to the many project leads, PEM Outreach and PEMCET members, the Brand Center, and everyone else who assisted in developing 19 Riley EM best practices over the last few years. This work will continue to impact thousands of pediatric visits to our IU Health EDs and improve pediatric care across the state.

Final Project Updates:

  • Pediatric best practice algorithms have been developed for both DKA and Headache. The Headache Algorithm visual is available here. Minor updates to the DKA visual are pending; an email notification will be sent once available.
  • Some minor changes were made to the Pediatric Sepsis algorithm. Please access the updated visual via the links below and replace any previously printed sepsis visuals to ensure the most updated version is available.

CDC Recommended Immunization Schedules

MMWR has published two reports summarizing 2024 updates to CDC Immunization Schedules:

As a reminder, the 2024 Immunization Schedules for Clinicians were published in November 2023.

The 2024 Child and Adolescent Immunization Schedule includes new recommendations for:

  • respiratory syncytial virus (RSV) vaccine,
  • respiratory syncytial virus (RSV) monoclonal antibody,
  • Mpox vaccine,
  • 20-valent pneumococcal conjugate vaccine, and
  • pentavalent meningococcal vaccine.

The 2024 Adult Immunization Schedule includes new recommendations for:

  • respiratory syncytial virus (RSV) vaccine,
  • Mpox vaccine, and
  • pentavalent meningococcal vaccine.

Both schedules also include updated recommendations for 2023-2024 formula COVID-19 vaccine, influenza vaccines, and inactivated poliovirus vaccine (IPV).

Nursing mothers: AAHC has 15 dedicated lactation spaces

 

The IU Health adult academic health center provides dedicated lactation spaces for breastfeeding mothers. These private spaces are distributed throughout IU Health Methodist and University campuses and are available to team members and visitors.

At IU Health Methodist Hospital, there are three rooms located in public access areas that are available to all, including team members and visitors. There are an additional six spaces available to team members that are located in employee-only access areas across the A, B and C Buildings, and one in the Neuroscience Center.

At IU Health University Hospital, there are two rooms located in public access areas that are available to all, including team members and visitors. There are an additional four spaces available to team members that are located in employee-only access areas in the AOC, Simon Cancer Center and University Hospital.

If you are looking for a lactation space, please speak to your leader to help identify the space most convenient to you. Leaders have a list of the dedicated rooms and a contact for gaining team member access.
