CMO Updates from Michele Saysana, MD

On Friday, April 26, the new academic health center planning team hosted a monthly update for leaders at the AAHC. You can watch brief video clips on Connect for updates on construction and the South Support Building, which will be our state-of-the-art logistics hub on the new campus.

Clinical Council Updates

Critical Care Council

Quality/ Care Variation:

• Incentive Spirometry: As part of a large respiratory-led initiative to improve respiratory therapist workflow and ensure timely care for necessary patients, the majority of the council approved the proposal to un-check orders for incentive spirometry within order sets owned by Critical Care/Pulmonary. The order will still be available within the applicable order sets, providers will now need to assess the need on a case-by-case basis.
• Sepsis Care: Dr. Ellen Klink (Hospitalist) and Program Manager Rachel Knapp, leads of the System Sepsis Steering Committee, provided an update on work to improve sepsis care. The main takeaways were that the system CMOs have approved 2024 goals for nursing and providers.
• Provider goal: 60% inpatient sepsis order set usage for all patients with inpatient sepsis diagnosis.
• Nursing goal: 65% of sepsis screenings occur within 15 minutes of alert firing on units with the Cerner Alert and Screening Process.
• Access system tracking dashboard: InpatientSepsis - Power BI
• Action Item: Council members are asked to spread awareness of this initiative and encourage the use of the inpatient sepsis order set.

Pharmacy:

• Council approved proposed updates to the Alcohol Withdrawal and Vasoactive Infusion Medication PowerPlan.

Infectious Disease Council

• Required ID Consults Update: Riley Hospital and the Maternity Tower will be implementing required ID consults for patients with blood cultures testing positive for Staph aureus, Candida, and Enterococcus following the implementation of similar measures at University and Methodist hospitals. The anticipated go-live date is June 1, 2024. 

Newborn Council

Quality/Care Variation:

• Neonate Quality Metrics: The Council reviewed system performance of quality metrics for Q1 2024.  The council noted no significant changes to previous quarters.  The data for 1/01/2024 - 3/31/2024 is as follows:
• (NICU and Baby Mortality on Encounter excluded for all measures; Birth weight greater than 2000g for Hep B):
  • Vitamin K Administration
  • System Q1 = 96% (Goal – 98%)
  • Hepatitis B Administration
  • System Q1 = 90.88% (Goal – 93%)
  • Erythromycin Administration
  • System Q1 = 97.46% (Goal – 98%)

NICU Council

Quality/Care Variation:

• NICU Harm Dashboard: The council reviewed where to input data for the NICU harm dashboard, reminded members to update the data monthly, and answered questions about how safe sleep audits were done.
• New Scrubbing Technique: The council reviewed the new scrubbing technique used at Riley for entering the NICU.  Riley now uses Avagard, a surgical hand antiseptic.  The new process excludes the use of a scrub brush, which can disturb the skin and create opportunities for bacteria.
• Prevantics for IV Access and Central Lines: The council discussed using the Prevantics swabs for skin and device preparation.  Prevantics is 3.15% CHG and 70% isopropyl alcohol and comes in two different packages, purple for use on people and green for use on devices.  The swabs can be used on patients born greater than 28 weeks and those born less than 28 weeks but are now at least 2 weeks old.  Riley has seen and studies have shown the use of Prevantics significantly lowers CLABSI and is significantly more effective at disinfection of needleless connectors.
• SecurePortIV:  The council discussed best practices for the use of SecurePortIV, a catheter securement adhesive used to secure both the catheter to the insertion site and the hub to the skin.  SecurePortIV reduces catheter movement, migration, and dislodgement and seals the insertion site.   
• LOS Documentation Tips and Tricks:  The council reviewed Vizient data on expected LOS and expected Mortality for the NICUs.  The council discussed concerns with the data and how to make it useful and meaningful.  The council noted the Vizient data wasn’t perfectly reflective of NICUs.  Dr. Rose gave council members tips and tricks to improve documentation resulting in a higher expected LOS.  The presentation focused on Severity of Illness Score, optimizing coding, various conditions, and steps for documenting the problem list.       
• Small Baby Protocol:  Dr. Lien briefly shared they have updated the Small Baby Protocol.

Clinical IS & Technology:

• NICU Cooling Order Form:  The council completed a five-year review on the NICU Cooling Order Form.  The council voted to approve the updated version which replaces esophageal with rectal temperatures and removed the chest x-ray order. 
  • DECISION:  The council voted to approve the updated version of the NICU Cooling Order Form. 
• Neonatal Abstinence Syndrome PowerPlan:  The council reviewed the Neonatal Abstinence Syndrome PowerPlan that was used six times in 2023.  The council voted to retire the PowerPlan, and the recommendation will go to Newborn CE Council.
  • DECISION: The council voted to retire the Neonatal Abstinence Syndrome PowerPlan

LiveHelpNow Pilot in Cerner

Beginning in April, providers have access to live support directly in Cerner. During the three-month pilot of this tool, only team members included in the pilot will see the new LiveHelpNow button available in the Cerner toolbar.  

With this tool, Informaticists are available 24/7 to take live questions, troubleshoot and guide task completion as you move through the electronic medical record. If providers find this resource valuable, the feature could be extended indefinitely and expand to all electronic medical record users.  

Providers and pilot participants are encouraged to test the feature and provide any feedback to IUHMGLeadership@iuhealth.org.

DynaMedex Replacing UpToDate

DynaMedex will replace UpToDate as the point-of-care medical resource for IU Health. DynaMedex was carefully vetted by a systemwide group of physicians, advanced practice providers and pharmacists. The Ruth Lilly Medical Library at the IU School of Medicine is also transitioning to DynaMedex in May. 

Both UpToDate and DynaMedex are available to team members through Sunday, June 30. UpToDate will no longer be available after June 30*. Continuing Medical Education (CME) credits will need to be downloaded from?UpToDate?before Sunday, June 30. 

DynaMedex and UpToDate are both currently accessible through the Methodist Medical Library. After Monday, June 10, DynaMedex will be accessible in Cerner and UpToDate will be accessible in the Methodist Medical Library.
